Russell Bryant d1f1b3c4d6 * Add channel locking around datastore operations that expect the channel ...

to be locked.
* Document why we don't record Local channels in the dialed interfaces list.
* Handle memory allocation failure.
* Remove the dialed variable, as it wasn't actually needed.
* Tweak some formatting to conform to coding guidelines.

Finished up a previous fix to overcome a compiler warning; the app NoCDR() has been updated to mark the channel CDR as POST_DISABLED instead of destroying the CDR; this way its flags are propagated thru a bridge and the CDR is actually dropped. The cases where only one channel in a bridge has a CDR was cleaned up.
